10th grade in-person class

Hari OM 10th graders!

To make sure we are all in the safest environment possible when we meet in Chinmaya Haridwar for our Bala Vihar class, each student coming there in-person, please attest to the form below.

Please bring in your vaccination card so we can note it for our next in-person classes.

By signing up, I attest that:
1.	I am fully vaccinated.
2.	For the last 14 days, my family has not been in close proximity with someone with COVID-19 or traveled abroad. I will notify CMP if this status changes.
3.	I will observe masking and social distancing guidelines given to me by CMP.
